I found an opinion/blog piece here that says there is a high school student responsible, so I'm not 100% sure who is responsible.

If they were "filler" names why were they so specific? Why not Joe Smith, Anne Jones, etc.?

However, an e-mail to a writer for the San Gabriel Valley Tribune claimed the names were “filler text,” used when the yearbook was being designed. The names were supposed to be replaced, the writer said, but the page was never corrected and the mistake got through a proof reader.
That is rather vague. "an e-mail"? From whom? It doesn't even say a company official.
Obviously it is the work of at least one of the students who worked on the yearbook.
The company needs to figure it out and release a statement to that effect, unless they want to be held liable for any further actions on the part of the rightfully offended parties.
Actually at this point even if they did figure out which kid did it, they would probably still face some civil suit or at the very least having to print up new yearbooks for free. That would be fair, it may have been the work of a student, but there should have been some sort of verification process. Let the yearbook company make restitution and have the them sue the family of whichever a*****e kid thought he was being funny.

I saw this story on the news the other night. It is so wrong. How about the white guys/gals names be changed to Bubba and Mary-Sue. That list could go on and on as well. Well maybe not, we might start running into Bubba Jr.
