There's alot of controversy on the results of this study but the FDA maintains that there isn't enough data to be 'safe' with it. Some people have research that says it's a mutagen, others claim that THAT data is false and THEIR data says it's safe.

If I was in the FDA I would be reluctant to approve a product that has any controversy over safety. It's easier to be cautious and say No to something like a sweetener than to be wrong and allow people to harm themselves with a product that the FDA has said was safe.

Then again, the WHO released a report (PDF, 18MB) in 2006 claiming that stevia is safe:

"stevioside and rebaudioside A are not genotoxic in vitro or in vivo and that the genotoxicity of steviol and some of its oxidative derivatives in vitro is not expressed in vivo."
The part about stevia starts at page 124 of the PDF file.
