Injecting saline into your body just can't be that good for it... especially in such high amounts that just get absorbed...

What happens if they get into a fight and their lump winds up getting punched? Does it pop? does the saline shift to the other side? Could it blind them or dislodge their eye?

Too risky for my taste...

I can see they must be huge fans of the Elephant Man.

The saline just absorbs into the body. This does not last too long. However I would think the skin would be stretched out and would laugh & laugh & laugh to see the big droopy flaps of skin hanging on their faces.

Those big droopy flaps of skin are the reason it's done medically, in select patients who need large skin grafts (burns, reconstructive surgery, etc). The skin is stretched/expanded in the exact same way and harvested for the intended use.
