It was sculpted by Peter Schipperheyn, and it's in Mt Macedon Cemetery, Mt Macedon. Victoria. Here's the sculpture in his gallery.

That is incredibly beautiful. If I were planning to be buried* one day, that's what I'd want as a monument. Truly a work of art.
*I'm planning on being cremated, not living forever. Hence the lack of a proper burial.
