I couldn't find the stats, but there are many states that don't require coverage of birth control, even if it's prescribed for a medical reason other than preventing fertility. I showed up at every single insurance meeting when I worked for the State of Wyoming to protest that. The department head learned to hate me--particularly after he joked about paying for Viagra but NOT birth control.

"So, sir, you think it's a joke to pay for someone to knock up a gal, but NOT pay for her to avoid that?"

In 2004, a Guttmacher Institute study showed that 86% of private insurers covered a range of contraceptives. Which sounds really good, but this figure excludes all self-insurers and government plans. Interestingly, it also showed that coverage was better in states that had mandates. At the time, 30 states had no mandate.
