"We are extremely concerned about this continuing pattern," Director of Health David R. Gifford said in a written statement. "While the hospital has made improvements in the operating room, they have not extended these changes to the rest of the hospital."

It never said whether it was the same doctor or not...if it was then he should have his medical degree striped for neglegence. Or it could be a computer problem, like a bug in the system.

The mistakes were made by three different doctors it says.. Same doctor or not tho, it absolutely boggles my mind. I'd think the neurosurgery program would be ordered shut down - at least temporarily - in addition to the fine and reprimand. We're talking grievous and multiple errors here, good grief!
